Motor failure, what next?

Aerotech i280 DMS. Ring snapped off the bottom under thrust, despite applying a thick layer of CA. Rocket heavily damaged. Next steps:
fill out a MESS form
fill out an AT warranty form
post pictures here

Which ring did it have, paper or 3D printed?

We had a college launch in the Spring and the motors had the 3D printed ring. All motors worked fine.

I remember an addendum to the instructions in a DMS I used last year that discussed thrust ring reinforcement. Did yours have the same, and were they followed?

Personally, I feel more comfortable with an RMS that I build myself in a metal casing, and I never had a failure. So, I don't use DMS or SU very often.
