Last Saturday I flew an E11 in my Estes Dude.
When I was assembling the motor, I removed the forword closure after it was assembled to reposition the igniter. When I was putting the closure back on, I couldn't find the Forward O-ring, so I got another one from the bag and put it in. I screwed the closure on and flew it.
It burned just fine, but when I got home and cleaned the motor, ou popped two forword O-rings. Oops... I must have not seen the first O-ring sticking to the forward closure when I put it together again. The case is fine, but I thought Joe Burger and some others might be interested.
And a lesson was also learned, always check twice before getting another O-ring
