Lab Rat Rocketry makes some, including the ones that's included with the Apogee Easy Mount. Haven't heard of any issues with them, the trick is that you have to make them thick enough to handle the pressure without shattering.
 
I printed a small one into the face of a donut av-bay I made. Holds about 1g of BP. Made the well a parabola, thinking it might focus the blast more forward out of the printed part. Walls are ~3mm thick carbon fiber PETG. It's survived 4 test fires so far.
 
I have printed 1000s to test filament. They work well and will hold up to charges. I have some already designed if you are interested.
