There were a number of recovery issues but so far as I know, just one CATO - an M1845 at altitude just over two seconds after first motion, on a Wildman Intimidator 5 (I believe) flown by a team from Clark College. This motor has a broad thrust peak between 1 and 2.25 seconds and has had a previous failure (per MESS) in this general timeframe.
