At the CMASS launch Saturday I had two instances of ejection failure. Both involving Estes D12-3.
My Acme Spitfire came in ballistic destroying the nose, but no other damage.
The twin engine 1912 Gunpowder had one engine ejection charge fail to go off, the other went off, but so weakly that the nose and chute failed to separate. Fortunately it has a foam nosecone which absorbed most of the impact. Minor crush of the body tube, so its an easy cut and splice. Still really annoying.
