Another good book is "Truth, Lies, and O-rings" by Allan McDonald. I worked as an aerothermal analyst in engineering at Thiokol 2 years before the accident and many years after the accident. All the upper management in Thiokol and NASA that were directly involved in the accident were eventually removed or eliminated within a short time after the accident.
