Can You ID This??

I came across a small bag on my workbench shelf that seems to contain a pyrogen kit I must of ordered some time ago (See Pic). I did an Internet search but could not find anything that looks like it.

I don't find any instruction for its use and was wondering if anyone recognizes this product? Can you supply me with the instructions for its use?
